Birth Certificate in Gurgaon/Gurugram is made in three steps. The first step is collecting an acknowledgement of birth report from the hospital; the second step is visiting the Municipal Corporation Gurugram’s office within 30 days of birth for the issuance of without name birth certificate. The third and final step is visiting Municipal Corporation Gurugram’s office again to apply for the name addition of child in without name birth certificate.

PROCEDURE OF BIRTH CERTIFICATE IN GURGAON/GURUGRAM
STEP 1: ACKNOWLEDGEMENT OF BIRTH REPORT
When your child takes birth in a hospital, the hospital informs the local registrar of births & deaths about the birth of your child. The hospital informs this online through a centralized software by filling out a form.
After informing the registrar, an Acknowledgement of Birth Report gets generated, which will be given to you by the hospital.

STEP 2: ISSUANCE OF WITHOUT NAME BIRTH CERTIFICATE
After the receipt of The Acknowledgement of Birth Report, parents need to visit Municipal Corporation Gurugram within 30 days to get the registration approved and a birth certificate generated by submitting an application for issuance of birth certificate with the below mentioned documents to the customer facilitation center of MCG.

Joint Affidavit of Parents’ (Notary Attested)
Filled Form (To be collected from Municipal Office)
After submission, either they will issue the birth certificate same day or they will tell you to visit after a few days. The certificate issued by them will look like this.
This is called Without Name Birth Certificate. It is complete in every aspect except the name of the child.
For name addition in it, parents’ need to follow the Name Addition process explained in step 3.
STEP 3: NAME ADDITION IN WITHOUT NAME BIRTH CERTIFICATE

Procedure in detail:
Collect the Name Addition Form from the inquiry counter of Municipal Corporation Gurugram.
Fill it completely with all the details. Please be careful while filling up the form.
Visit the local tehsil office for a joint affidavit of parents attested by a notary.
Attach copies of parents’ Aadhaar
Attach the copy of without name birth certificate
Obtain a token from the customer facilitation centre of Municipal Corporation Gurugram
Submit the file in your turn.
Collect the receipt after submission.
Visit after 40 days for certificate collection.
IMPORTANT THINGS TO KEEP IN MIND WHILE COMPLETING THESE STEPS
Please avoid visiting the MCG office by four wheelers as there is no parking space for four wheeler.
For token, please reach by 8:00 a.m. As limited tokens are distributed daily.
Submitting the file would take 4-6 hours. So be ready to stand in line for that duration.
No changes will be entertained in the child’s name in the future, so be careful with the spelling of the child’s name.
Please write the child’s name in capital letters with adequate spacing, computer clerks often make mistakes in spelling. If they make an error in yours, then be ready for nightmares.
