The birth certificate of a child born in a private hospital in Haryana is made in three steps. First is the receipt of acknowledgement of birth report from hospital. Second is the receipt of “Without Name Birth Certificate” from local Birth and Death Registrar. The last step is the addition of name of child in it.
The name addition application will be submitted to same local Birth and Death Registrar from which you received “Without Name Birth Certificate”. In Haryana there are 600+ Birth and Death Registrars.
If you know your registrar, then it is good else there is another method to determine the registrar. Just look at the top of the “Without Name Birth Certificate”.

Collect the Name Addition Form from your local registrar’s office. (Applicants living in cities having Municipal Corporation will have to submit application to Customer Facilitation Centre of their Municipal Corporation Office). Fill it completely with all the details. Please be careful while filling up the form.
Visit the local tehsil office for a joint affidavit of parents attested by a notary.

No changes will be entertained in the child’s name in the future, so be careful with the spelling of the child’s name.
Please write the child’s name in capital letters with adequate spacing, computer clerks often make mistakes in spelling. If they make an error in yours, then be ready for nightmares.
